Supporting the development of a consistent approach to the implementation of the new Public Health functions as it transfers into Local Government as well as stimulating innovation and promoting the Year of Health & Wellbeing.

Delivery for this programme has been commissioned from North West Employers and key partners and will focus upon providing dedicated support for elected members and chief exectives during the transition and developing workforce strategies and support.

Personalisation
This programme is being commissioned to support the personalisation of health and social care budgets and to facilitate the development of stronger more patient-focused markets in independent living and public health.

Holding the Ring 11/12
This workstream will ensure that key priority policy areas are kept in mind during a period of transition.  It is essential that issues like safeguarding and dementia are not only not forgotten, but continue to be developed.

Social Care Integration around services for frail older people.

Building on the NHS funding transfer to social care & the development of reablement services, this project will support local whole system partnership and integrated working utilising health & social care benchmarking data, focussed on frail older people, and regional learning and practice exchange networks to inform and support local system improvement.
